preventative recommendations following the findings of safety

potential safety threats through audits and hazard reporting (often
labeled ‘quality management’).
Historically however,
many airlines have inherited separate safety and quality management
systems leading to a fragmented view of safety and quality management.
This in itself is a risk, but one that can be reduced with integrated

Air Safety reporting
is a vital component in our total commitment to flight safety. The
simplified and dedicated Air Safety Report (ASR) form has been produced
to expedite the completion, handling and “filtering” of reports.
Flight Manager
Solutions Air Safety Reporting system is capable of tracking a variety
of incidents classified in any of the following broad categories.
The program has been
built using the FAA’s System Safety discipline. That discipline is
defined as the application of special technical and managerial skills to
the systematic, forward-looking identification and control of hazards
throughout the life cycle of a project, program, or activity.
The primary objective
of System Safety is accident prevention. Accident prevention can be
achieved by proactively identifying, assessing, and eliminating or
controlling safety-related hazards, to acceptable levels. A hazard is a
condition, event, or circumstance that could lead to or contribute to an
unplanned or undesired event. Risk is an expression of the impact of an
undesired event in terms of event severity and event likelihood.
Throughout this process, hazards are identified, risks analyzed,
assessed, prioritized, and results documented for decision-making. The
continuous loop process provides for validation of decisions and
evaluation for desired results and/or the need for further action.

Assessment
Risk Assessment is
generally defined as the process of combining the impacts of risk
elements discovered in risk analysis and comparing them against some
acceptability criteria. Risk Assessment can include the consolidation of
risks into risk sets that can be jointly mitigated, combined, and then
used in decision making.

Reporting Form
This screen shot of the Air Safety Reporting form is included in Flight
Manager Solutions. Its typical use of for operations personnel to
view data collected for the relevant incident. In addition to the
in-house system, Flight Manager Solutions comes with a .dll and
executable that can be installed on your Windows based web server with
an internet share to the databases used by Flight Manager Solutions.
This system will allow your personnel to file a report from anywhere in
the world via the web and avoid the time lapse it takes to obtain and
track reliable data. Data can be directly entered into the system
via a workstation if the web component is not used.
